Clearance Process

The University of Florida COM/ Jacksonville now requires all students rotating on our campus to submit a completed clearance form prior to departing the campus. Copies of this form should be provided by the individual department in which you are rotating. On the final day of the rotation, students who resided in the dormitory should turn in keys, meal card, and parking card to their department student coordinator, and complete a dormitory evaluation form. All other items loaned by the department should also be returned, and a signature from the department student coordinator should be obtained on the clearance form. Medical students and PA students will also be required to complete a rotation evaluation.

College of Medicine students may have their grades withheld if this process is not completed.

During completion of the checkout procedure, please leave your car in the student lot - you do not need an access card to exit this lot. If you move your car to a garage or on other on-campus parking location, you may be ticketed and/or incur a parking fee.
