To refer a child to the Jacksonville Cleft Lip and Palate Clinic, call (904) 633-0935.
You will need to provide the patient's name, address, and phone number so the clinic can contact the family to conduct a pre-clinic screening, provide needed support, information and literature, request appropriate medical records, and make an appointment for the patient's initial team evaluation.
If you prefer, you can instead complete the referral form and fax it to (904) 633-0921 , Attention: Cleft Lip/Palate Clinic.
