Goals
To learn a logical, concise, thorough, yet cost-effective, approach to the care of the acutely ill hospitalized oncology patient.
To learn the cognitive skills necessary for the care of the acutely ill and hospitalized oncology patient.
To become proficient in the technical skills necessary to care for the acutely ill hospitalized patient and the chronically ill outpatients.
To learn to become a member and leader of the health care team in the provision of care of both inpatient and outpatient oncology patients.
To learn the appropriate and cost-efficient diagnostic studies for the evaluation of both inpatient and outpatient oncology patients.
To develop the clinical judgment necessary to prioritize patient problems and develop an appropriate treatment plan.
To learn to understand one's limitations in the care of one's patient and call upon consultants for assistance in patient care when necessary and appropriate.
To learn to be empathetic toward the patient and his/her family during his/her acute illness and hospitalization.
Objectives:
The fellow will learn the following objectives:
The etiology of cancer, including predisposing causal factors leading to neoplasia.
Fundamental concepts of cellular and molecular biology, cytogenetics, immunology, basic and clinical pharmacology, pharmacokinetics, toxicity and tumor immunology.
Etiology, epidemiology and natural history of cancer.
Diagnosis, pathology, staging and management of neoplastic disorders.
Immune markers, immunophenotyping, cytochemical studies and cytogenetic and DNA analysis of neoplastic disorders.
Molecular mechanisms of neoplasia, including the nature of oncogenes and their products.
Principles of gynecologic oncology.
Pathophysiology and patterns of tumor metastases.
Indications and application of imaging techniques in patients with neoplastic disorders.
Chemotherapeutic drugs, biologic products, and growth factors and their mechanisms of action, pharmacokinetics, clinical indications and limitations, including their effects, toxicity and interactions.
Multi-agent chemotherapeutic protocols and combined modality therapy of neoplastic disorders.
Principles, indications and limitations of surgery and radiation therapy in the treatment of cancer.
Principles of, indications for, and complications of bone marrow transplantation and peripheral stem cell harvests, including the management of post-transplant complications.
Technique of thin (or fine) needle aspiration biopsy.
Concepts of supportive care, including hematologic, infectious disease and nutritional.
Management of the neutropenic and/or immunocompromised patient.
Pain management in the cancer patient.
Rehabilitation and psychosocial aspects of clinical management of the cancer patient.
Hospice and home care for the cancer patient.
Recognition and management of paraneoplastic disorders.
Cancer prevention and screening.
Clinical epidemiology and medical statistics, including clinical study and experimental protocol design, data collection and analysis.
Participation in a tumor board.
Personal development, attitudes, and coping skills of physicians and other health-care professionals who care for critically ill patients.
Human immunodeficiency virus-related malignancies.
Specific Procedural Skills to be Acquired:
The program will provide fellows with the opportunity to develop competence to work effectively as part of a multidisciplinary team.
The program will provide the opportunity to gain competence or expertise in the performance and (where applicable) interpretation of the following:
Serial measurement of palpable tumor masses.
Bone marrow aspiration and biopsy and their interpretation.
Serial measurement of tumor masses by palpation or radiologic means.
Assessment of tumor imaging by computed tomography, magnetic resonance and nuclear medicine imaging.
Use of biologic and chemotherapeutic agents through all therapeutic routes.
Correlation of clinical information with the finding of histology and immunodiagnostic techniques.
