Rotations for External Residents/Fellows

Welcome and thank you for your interest in the University of Florida College of Medicine – Jacksonville (UFCOM-J).

If you wish to participate as a resident or fellow in an elective rotation experience with one of our clinical departments or GME programs, please note the following instructions:

  • Your program director's signature is required at the bottom of the external rotation request form and he or she is responsible for identifying the rotation and rotation-specific goals and objectives for your experience at UFCOM-J.

  • Please use this application form to submit the rotation request: External Rotation Request Form. Please note that there is a $25 non-refundable application fee for all applicants. You may pay by check or money order: please make payment out to UFJPI and mail with your request form or call 904-244-3149 for more payment options.

  • Plan for a timeline of at least 120 days from time you submit your request in order for UFCOM-J to 1) assess if your request can be accommodated, 2) to process agreements, and 3) to provide information necessary to permit you to start.
